Dear Patient:
Below, please find the necessary forms for your upcoming appointment with Dr. Anne Abrahamson. If you have any questions, please call us at (734) 287-3000. Or if you would like, any questions you have may be addressed at the time of your appointment. Filling these forms out ahead of time will help us to get you in and out in a timelier manner.
When you come in for your appointment, please bring your driver's license, insurance card(s), workers compensation information, or any auto information you have. Workman's compensation and auto cases must provide this information prior to the time of the appointment so that we can connect with the Case Manager in advance so we can best meet your needs.
Also, if you have any written radiologist reports, including copies of MRIs, X-rays, or any other doctor's notices addressing your injury, please bring them with you. We are unable to review imaging on discs; the written reports are satisfactory.
Paperwork should be completed at least 48-72 hours prior to your appointment in order for Dr. Abrahamson to review your case. These items can be submitted online, or printed and sent via fax, email, or hand delivered to our office.
